Application Steps
Before applying for DSPS services you must have applied to Cuesta College and have a myCuesta login.
Upload documentation verifying your disability using the DSPS Online Portal (AIM).
- Have a licensed professional complete the Verification of Disability Form and upload it to your AIM account.
- If you are an incoming high school student, please upload to your AIM account a copy of your most recent:
- Individualized Education Plan (I.E.P.)
- Psych-Ed Evaluation or Triennial Report
- 504 plan
- If you are a Veteran, upload a copy of your Veteran's benefits that details your disability and percentages.
- Upload documentation from Department of Rehabilitation that contains information on your disability.
- Upload a report from your licensed medical provider that states your disability, suggested accommodations and has a signature from the medical professional.
- Upload other documentation as directed by a DSPS specialist or DSPS director.
If the documentation submitted does not have all the required information in it, you will receive an email requesting additional information.
Optional: Allow DSPS to communicate with someone other than you about your DSPS information
If you would like DSPS staff to be able to communicate with someone other than you about your DSPS information, you must complete a DSPS FERPA Form.
The DSPS FERPA Form is different than the general Cuesta College FERPA form. For general information about FERPA, please go to Family Education Rights and Privacy Act.
After completing the application and uploading your documentation, the DSPS office will contact you through email to set up an intake appointment. Currently, intake appointments are done on the phone or through ConexEd video meetings.
