This form is needed if you would like DSPS staff to be able to communicate with someone other than you about your DSPS information (also available in PDF format).

Authorization to release information relating to services provided by DSPS while enrolled at Cuesta College. Please email completed form to dspsinfo@cuesta.edu

Must be completed and signed by a Licensed Professional. Other qualifying documents are listed on our Application Steps page.
