Cuesta College Accident Reporting Procedures 

For Employees and Volunteers 

 

MEDICAL EMERGENCIES: immediately call 911 or Public Safety. Contact your supervisor after emergency services.

NON-EMERGENCY:

If medical treatment is needed or requested: call the Nurse Triage line at 1-855-519-8472. The Nurse Triage Line (available 24/7) will help determine what your next steps for seeking medical treatment. After receiving direction from the Nurse Triage Line contact your supervisor and work together to complete the Accident Investigation Report

Employees who live outside of California AND not in Washington State must contact 1-888-749-1950 to report. Additional information for employees who live outside of California is available here

Employees who live in Washington State must work through the Washington State Department of Labor and Industries. 

If medical treatment is declined: the employee completes the Declination of Medical Treatment form and returns a completed form to HR. Notify your supervisor and work together to complete the Accident Investigation Report

NEAR MISS: contact your supervisor to complete the Accident Investigation Report

SAFETY HAZARD: if you see something, say something. Hazards are accidents in the making – please help us prevent accidents before they happen. Contact your supervisor to report a safety hazard.
