Tutorial: How to navigate the Student Health Portal
Our clinic provides assessment, examination and treatment of short term/acute illness and minor uncomplicated medical problems; reduced cost antibiotic prescriptions; over the counter meds; and home care instructions. Some conditions may require a referral to an outside provider. Click here to learn more.
If you need special accommodations, a translator or an interpreter, please let us know in advance by email or phone.
Note: For follow-up appointments, you do not need to fill out intake forms again unless you need to update/change information, or it has been more than 1 year. Remember, forms for Mental Health appointments are different than Nursing consultation forms.
TB Risk Assessment student workers:
This is a telehealth appointment and is available on Tuesdays and Thursdays via Zoom.

TB Risk Assessment as a requirement for your program/class, volunteer or work off campus:
First you need to verify with your program if you need a TB Risk Assessment or a TB Skin test (PPD).
This is a Zoom consultation. Please follow directions:

A TB skin test requires a visit with a health care provider. On the first visit the test is placed; on the second visit (48 hours after the administration of the test) the health care provider reads the test. If you need a two step you will repeat this process after your first test and read.
The TB skin test is performed by injecting a small amount of fluid under the skin on the lower part of the arm. A person given the tuberculin skin test must return within 48 to 72 hours to have a trained health care worker look for a reaction on the arm.
Cost: $10 for Cuesta College Students (cash, check or charge to student account) TB/PPD tests are not able to be scheduled online. Please contact us to schedule your in-person TB skin test.

How to meet with your health provider on Zoom?
The Student Health Center is currently offering telehealth services. Your Zoom link will be available at the top of your health portal home page 15 minutes before your appointment.
To access: https://cuesta.studenthealthportal.com, and log in using your student credentials.
Click "start your meeting now" and meet with your healthcare provider.
If it is an in-person appointment, you will also receive a notification confirming the time, location, and safety recommendations.
The student health portal will let you cancel your appointment online 24 hour hour before your meeting.
Log in to the portal using mycuesta credentials: https://cuesta.studenthealthportal.comGo to appointment scheduling, view appointments, upcoming, and you will see the option to cancel. If you try to cancel in less than 24 hours, the system will not allow you to do it online. Call us at 805-546-3171 or send an email to: studenthealthcenter@cuesta.edu

Telehealth services Form: Consent to receive TeleHealth services with the Student Health Center (SHC) staff and providers.
Consent for Minors Waiver: If you are a student and are under the age of 18 this waiver must be filled out prior to any physical or mental health appointments at the Health Center.
Authorization to Release or Disclose Patient Information: All appointments at the Health Center are confidential, however, if you would like to release information about your sessions and appointments to someone or an entity this waiver must be filled out.
Translator or interpreter authorization: Students may request a translator/interpreter to be present during their in-person and/or virtual sessions with health providers so that they may communicate effectively.
Acknowledgment of Confidentiality by Translator/Interpreter: Translators and interpreters must sign this form prior to the appointment to ensure that patient information remains confidential.
Appeal form for mental health therapy: Not showing up for an appointment is considered a cancellation. Less than a 24-hour notice requires filling out an appeal form. Please review our policy.
Authorization to release TB Risk results to HR: The tb risk for a new hire is a Zoom consultation required by Human Resources and you will need to begin by completing this form allowing us to send your results to HR.
NEAT Intake Forms: If you are scheduling an appointment with our Nutrition Education Action Team, please bring these forms filled out to the Health Center no later than 48 hours before your appointment.
Student Accident & Injury Report: In the event of an accident, injury or illness on campus, a student accident report must be submitted to the Student Health Center. Please fill out the student accident report right away. Afterwards, a student health representative will contact you.
