Student Conduct Incident Report Form/Student Support Referral Form
All students attending San Luis Obispo County Community College District at any campus site and when representing Cuesta College in any off-campus activity, assume an obligation to conduct themselves in an acceptable manner compatible with the Student Code of Conduct and Academic Honesty. We know that many more students with mental health diagnoses and psychotropic medication needs are entering college, more students are accessing mental health services on campus, and more students are presenting with serious psychological issues than in years past. This sometimes presents a challenge for instructors and college personnel.
The Student Mental Health Program, collaborating intensively with the Office of the Vice President of Student Success and Support Programs, has developed a new Student Code of Conduct Incident Report Form and a separate Student Support Referral Form. It is our goal, through these new forms, to allow reporting parties to describe not only Student Code of Conduct violations but also student mental well-being concerns with the hopes of getting students the help they may need sooner than later and preventing potential problems.
Psychological distress can also compromise students’ academic achievements affecting their ability to stay in school. As a staff and/or faculty member you are in a position to identify and offer resources to a student in need.
The At-Risk Online Interactive Trainings are designed for staff and faculty to learn techniques for identifying and approaching distressed students in order to take appropriate action, such as making a referral to the school personal counselor. The three available trainings are: At-Risk- for the general student (45 min.), Veterans on Campus (30 min.) and LGBTQ on Campus (30 min.). At-Risk Online Interactive Trainings are free, accessible from any computer with Internet access, and can be completed in about an hour. Training need not be completed in one session; simply logout to save your session and log back in to continue later.

FAQs for Student Code of Conduct
This should be submitted within 24 hours of when the incident occurred, if possible. This allows for a quicker response time from the Office of Student Success as well as addressing the concerns in a more equitable time frame. Without proper documentation, the Office of SSSP will be unable to address the concerns that may be had with the student.
If you were able to resolve the concerns during the incident (i.e speaking with the student if possible, providing a failing grade for potential academic dishonesty) and you feel no further action is needed from the Office of SSSP, you can choose to submit the Conduct Report under "Information Only". You are also encouraged to contact the Student Support Resolution Coordinator (SSRC)/Division Chair/Dean if needing further support.
Submit a Report, only if you have confirmation the student cheated. Speak with the student first about your concerns so they can provide their input and address the allegations of academic dishonesty.
FAQs for Student Support Referral Form
This form should be completed only IF the student has agreed to receiving more support/services. This means it is strongly recommended to speak with the student about concerns that were raised prior to submitting this form. The reason for this is so students do not become overwhelm if they are contacted by the Student Support Resolution Coordinator. This allows for a better connection with the student and SSRC and creates that wraparound support. Contact the SSRC if you need further support.
SSRC will follow-up with the party who submitted the referral in an attempt to gather supplemental information and discuss supports that could be beneficial. SSRC will then follow up with student and depending on outcome of the conversation/meeting SSRC will provide an update to the reporting party of what resources were provided. SSRC will be unable to disclose any other information that is discussed during the conversation to respect confidentiality. This is not a punitive/disciplinary process, but an attempt to aid the student in connecting with resources.
