How to Apply for Disabled Student Programs & Services (DSPS)

 

1. Fill out a DSPS Application Packet and print it out 
   
                           OR

Stop by either campus to pick up a DSPS Application Packet:

San Luis Obispo Campus: Bldg 3300
North County Campus: Bldg N3100, Room N3121

 

• Have a licensed professional complete the Verification of Disability Form


*If you are an incoming High School Student, please attach a copy of your most recent

  • Individualized Education Plan (I.E.P.) along with your
  • Psycho-Ed Evaluation or Triennial Report

 

Note: A complete DSPS Application Packet includes the following:

  • DSPS Application for Services
  • Consent for Release of Information
  • Verification of Disability Form (VOD) with pertinent reports

 

2a. Return your completed application packet to either one of our DSPS Locations:

  San Luis Obispo Campus: Bldg 3300
  North County Campus: Bldg N3100, Room N3121

2b. Mail your completed application packet to either one of our DSPS Locations:

  North County Campus: 2800 Buena Vista Dr; Paso Robles, CA 93446

2c. Or FAX your completed application packet to either one of our DSPS Locations:

3. Once you have turned in your DSPS Application Packet, a Disability Specialist will review your application packet. 

4. A letter regarding your eligibility will be sent to your MyCuesta email address
